What if I make mistakes?

And your Health Profile is not the official record - a practitoner always keeps a record, whether you do or not. But your Health Profile can be a big help to both of you.
I'm really healthy, so I don't need it.
It still helps to have a record. If your health changes, you're ahead of the game. And the sooner you collect information like Family History, Past History, Immunizations and so forth, the easier it is.
